Follow-Up Testing Requirements

DOT Follow-Up Testing is a REQUIRED component of the Return-to-Duty process and is NOT OPTIONAL. Participants are responsible for remaining available for and completing all testing required under their SAP-prescribed Follow-Up Testing Plan.

Failure to remain compliant with required follow-up testing may affect your DOT compliance status and eligibility to perform DOT safety-sensitive duties.

 

Payment Policy

To ensure uninterrupted administration and scheduling of your Follow-Up Testing Plan:

• All invoices must be paid on or before the stated invoice due date.

• Payment extensions will no longer be provided.

• Invoices remaining unpaid 5 days after the due date will be cancelled.

• A $25 reinstatement fee will be assessed to reactivate services following cancellation.

• The outstanding invoice balance and applicable reinstatement fee must be paid before services can resume.

• Reinstatement of services does not eliminate or extend any testing requirements established under your Follow-Up Testing Plan.

 

Participant Responsibility

Participants are responsible for monitoring their invoices, making timely payments, maintaining current contact information, and completing required testing according to their Follow-Up Testing Plan.
